Preschool Registration |

In this section you’ll find the required forms and process in order to complete the Preschool (K3/4) Registration. The Preschool Admission and Registration Process is complete upon the school’s receipt of all of the following: 1. Statement of Faith signed by at least one parent or guardian Since faith is the foundation of our school, it is essential that each family understand these principles. Please take time to read our Statement of Faith, prayerfully consider them, and sign the Statement of Faith form (in the Preschool Registration Packet) to indicate your agreement. 2. New Student Application/Contract completed and signed This is contained within the Preschool Registration Packet. 3. Copy of Health and Immunization Record or exemption letter NEW STUDENTS—It is required that your child have an updated physical within six (6) months of the start of the school-year. Immunizations need to be up to date, as well. An Illinois Certificate of Child Health Examination form should be completed by your doctor, and a copy returned to MCS with your Preschool Registration Packet. RETURNING STUDENTS—Your form is already on file. Please just be sure immunizations are up to date. 4. $40 Non-Refundable Registration Fee Please enclose a check to Morris Christian School in the amount of $40, when you mail your Preschool Registration Packet to our office. |
